Healthcare Exploration and Careers Module Three Discussion

Rebecca went to the dentist for her 6 month cleaning. Within the waiting room, she noticed someone’s medical records were left in the lobby. She went to the receptionist to return the records, but the receptionist advised her to put them back and the patient will get them after their appointment.

What HIPAA violations were committed in this scenario? Should the dentist office be held responsible for the patient leaving their record? Discuss with your peers if and how the receptionist should be held accountable. Also discuss how the patient’s information might be used if stolen.

Aligning Money, Risk, Providers, and Politics

Respond to the post below , incorporate challenges you would anticipate for the proposals, as well as arguments to overcome those challenges. It should be 200 to 400 words with atleast two references APA format :

The U.S. healthcare system needs to make changes in order to benefit more people’s health. People are living longer with more co-morbidities and chronic illnesses. However, we also see individuals at a younger age living with the same chronic illnesses that could have been prevented with proper health screening. The progression of these chronic illnesses could be less rapid if individuals had the proper resources and quality of care they need to live a good quality life. For many individuals, it is too late to be receiving preventative care with their chronic illnesses. Proper screening of symptoms, risk factors and predispositions could have slowed or halted the progression of chronic illnesses. However, individuals may not have had a choice if they do not have proper access to affordable and quality healthcare.
If I could propose a plan, I would make it similar to the goals of the ACA’s expansion of Medicaid. I would make this a requirement for all states. Studies show that the ACA’s Medicaid expansion has prevented thousands of premature deaths in low-income individuals annually (Broaddus & Aron-Dine, 2019). In 2019, more than 2 million Americans fell into the coverage gap due to ineligibility for Medicaid and the state’s decision to not expand Medicaid (Garfield, Orgera and Damico, 2020). These are two million people who were unable to take advantage of primary or preventative care. Without proper screening, individuals miss the window of making lifestyle changes before the progression and diagnosis of a chronic illness. This then focuses on more expensive treatments to treat the disease rather than catching it early, resulting in more healthcare costs annually.
I understand that this proposed plan is easier said than done. The idea of Medicaid expansion has obviously been turned down before, primarily in southern states. I would do my best to advertise the importance of my Medicaid expansion-like plan. I would attempt to grasp the attention of minorities and individuals of low-income. However, I would also attempt to display findings in previous studies of the benefits of Medicaid expansion in the north to those who disapprove of the idea. I would then extend on that by illustrating the negative impacts everyone faces for those who arrive to the emergency room to get treatment that could have been prevented in primary care. Going to the emergency room for treatments would result in increased healthcare costs annually and take up a bed in the emergency room for someone who may really need it, resulting in increased wait times. Accidents can happen at any time and we all are getting older and watching out for loved ones who can get sick at any time. I figure this might be the best approach to grasp the attention of the opposers of a Medicaid expansion type of plan. I imagine this will attract many democrat’s attention. However, this may be more of a problem to attract the republicans vote. Again, I would advertise as much as I could to bring awareness to republicans that their healthcare and their loved one’s healthcare might be negatively affected by not expanding this proposed plan. I hope that would result in a lot of positive publicity, making my proposed plan sound the most beneficial plan for most individuals.

Safety score improvement plan paper

Write a 3–5 page safety score improvement plan for mitigating concerns, addressing a specific patient-safety goal that is relevant to quality patient care. Determine what a best evidence-based practice is and design a plan for resolving issues resulting from not maintaining patient safety.

Quality improvement and patient safety are health care industry imperatives (Institute of Medicine’s Committee on Quality of Health Care in America, 2001). Effective quality improvement results in system and organizational change. This ultimately contributes to the creation of a patient safety culture.

Consider the hospital-acquired conditions that are not reimbursed for under Medicare/Medicaid. Among these conditions are specific safety issues such as infections, falls, medication errors, and other safety concerns that could have been prevented or alleviated with the use of evidence-based guidelines. Hospital Safety Score, an independent nonprofit organization, uses national performance measures to determine the safety score for hospitals in the United States. The Hospital Safety Score Web site and other online resources provide hospital safety scores to the public.
Read the scenario below:
Scenario
As the manager of a unit, you have been advised by the patient safety office of an alarming increase in the hospital safety score for your unit. This is a very serious public relations matter because patient safety data is public information. It is also a financial crisis because the organization stands to lose a significant amount of reimbursement money from Medicare and Medicaid unless the source of the problem can be identified and corrected. You are required to submit a safety score improvement plan to the organization’s leadership and the patient safety office.
Select a specific patient safety goal that has been identified by an organization, or one that is widely regarded in the nursing profession as relevant to quality patient care delivery, such as patient falls, infection rates, catheter-induced urinary infections, IV infections, et cetera.
Deliverable: Safety Score Improvement Plan
Develop a 3–5 page safety score improvement plan.
Identify the health care setting and nursing unit of your choice in the title of the mitigation plan. For example, “Safety Score Improvement Plan for XYZ Rehabilitation Center.” You may choose to use information on a patient safety issue for the organization in which you currently work, or search for information from a setting you are familiar with, perhaps from your clinical work. Demonstrate systems theory and systems thinking as you develop your recommendations. Organize your report with these headings:
Study of Factors
Identify a patient safety issue. Describe the influence of nursing leadership in driving the needed changes. Apply systems thinking to explain how current policies and procedures may affect a safety issue. Recommendations
Recommend an evidence-based strategy to improve the safety issue. Explain a strategy to collect information about the safety concern. How would you determine the sources of the problem? Explain a plan to implement a recommendation and monitor outcomes. What quality indicators will you use? How will you monitor outcomes? Will policies or procedures need to be changed? Will nursing staff need training? What tools will you need to do this? Additional Requirements
Written communication: Written communication should be free of errors that detract from the overall message. APA formatting: Resources and in-text citations should be formatted according to current APA style and formatting. Length: The plan should be 3–5 pages. Font and font size: Times New Roman, 12 point, double-spaced. Number of resources: Use a minimum of three peer-reviewed resources.

Example of well graded paper attached to use as a reference.
